Job summary

Spectrum Health Systems, Inc in Norfolk, MA is seeking a Recovery Support Navigator to engage, educate and support patients seeking treatment upon release, and their families, to motivate and maintain engagement in community behavioral health services. This is a full-time, on-site role with Monday–Friday hours.

You will manage an assigned caseload, complete required documentation for Spectrum, DOC and licensing bodies, perform discharge planning for continuation of SUD treatment, and coordinate

Qualifications

• High school diploma required; LADC-II preferred.
• Bachelor's degree in human services or related field preferred.
• Case management experience strongly preferred.

Responsibilities

• Provide non-clinical services to engage, educate and support patients seeking treatment on release and their families to motivate ongoing engagement.
• Begin pre-release support that continues after release into the community.
• Manage a caseload and complete required documentation for Spectrum, DOC and licensing bodies on time.
• Discharge planning for initiation or continuation of SUDT services.
• Coordinate a re-entry package and document nasal naloxone receipt.
