Job summary

Endeavor Health is seeking a Registered Nurse for our LOH Main- Naperville location. This full-time role features a 3:00pm–11:30pm schedule, with responsibilities including assessing patients, planning and delivering care, documenting outcomes, and collaborating with care teams to ensure smooth admissions, transfers, and discharges.

Candidates should have a nursing degree and an Illinois RN license, with CPR certification and CPI training preferred.

Responsibilities

• Regularly performs duties of professional nurse which includes assessing patients' condition, developing and administrating patient plan of care, documenting care administered, and evaluating patient care outcomes.
• Collaborates with the Resource and Referral Department to facilitate patient admissions and transfers.
• Interacts with the patient, the patient’s family, and the appropriate health care providers to collect and analyze data regarding the patient’s physical, psychosocial, environmental, self-care, educational, and anticipated discharge planning needs.
• Assists in the development and attainment of nursing department goals and objectives; participates in activities to improve department functioning and to enhance own professional competence.
• Attends in-service programs and trainings; serves as a role model and central resource person for students; explains and clarifies organization policies and procedures to staff and patients; utilizes opportunities with community groups to further their knowledge and understanding of mental health problems.
