Job summary

Hospice of the North Coast is seeking a compassionate Licensed Vocational Nurse (LVN) to join our Palliative Care team in Carlsbad, CA. The position is part-time with after-hours shifts Monday–Friday 5:00pm–10:00pm, serving patients in their homes and coordinating care with an interdisciplinary team.

Responsibilities include skilled nursing visits, symptom management, patient and caregiver education, and documentation in the EMR.

Qualifications

• Graduate of an accredited Vocational Nursing program.
• Current California LVN license in good standing.
• Minimum one year of LVN experience (hospice, home health, palliative care, or medical-surgical experience preferred).
• Excellent assessment, communication, and critical thinking skills.
• Compassionate, patient-focused approach to care.
• Comfortable working independently while collaborating with a multidisciplinary team.
• Valid California driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Provide skilled nursing visits for palliative care patients in their homes.
• Assess patient needs and report changes in condition to the Registered Nurse.
• Administer medications and treatments within LVN scope of practice.
• Provide education to patients and caregivers regarding disease progression, symptom management, medications, and care techniques.
• Respond to after-hours patient needs, triage calls, and urgent symptom management visits.
• Collaborate with physicians, nurses, social workers, and the interdisciplinary team to ensure continuity of care.
• Complete accurate and timely documentation in the electronic medical record.
