Job summary

Central Coast VNA & Hospice is seeking a Licensed Vocational Nurse (LVN) to work under the direction of the patient’s physician and RN Case Manager to deliver individualized care and support the treatment plan. The LVN will promote comfort, independence, and quality of life for patients in a home health setting on a full-time basis.

Responsibilities include assessing patient status, reporting changes, administering medications, and coordinating supplies, while adhering to laws and agency

Qualifications

• Current California Vocational Nurse license.
• At least one year of LVN experience; experience in a home health, hospice, or applicable program preferred.
• Current CPR certification.

Responsibilities

• Assess and monitor the patient’s status and promptly report changes to the RN Case Manager or Clinical Supervisor.
• Administer prescribed medications and treatments in accordance with applicable laws and agency standards.
• Monitor and maintain appropriate medical supplies and coordinate replenishments as needed.
