The Elizabeth Hospice in San Diego County is seeking a part-time LVN to provide skilled nursing care under the supervision of an RN in patients' homes or facilities. You will work with the interdisciplinary team to ensure quality care across lifespan.

This role offers 16 hours per week with night shift 12:00 a.m.–8:30 a.m., weekend shifts, on-call one to two shifts monthly, and travel for patient visits. Competitive hourly pay and a comprehensive benefits package are provided.

Qualifikationen

• 12 months LVN experience minimum; 1–2 years in acute care/home health/hospice preferred.
• Current California LVN license
• Familiar with computerized bedside documentation
• Ability to function as part of a team and take direction from RN
• Phlebotomy certification a plus
• Bi-lingual (Spanish) a plus
• CA driver's license and proof of insurance required
• Excellent communication with patient and families and other medical professionals
• CPR/BLS certification current
• TB test results current

Aufgaben

• Assist RN in analyzing situation, anticipating problem areas and taking appropriate action.
• Evaluates patient response relative to outcomes in a safe, timely manner.
• Performs licensed nurse skills according to policies, standards of care, and competencies.
• Adheres to RN plan of care for the patient.
• Delivers services under supervision of RN and in collaboration with RN.
• Implements and documents teaching plans based on patient/family needs.
• Documents all care provided, including telephone interactions, using point-of-care electronic charting.
• Enters or retrieves data from established computer files.
• Orients, instructs and trains assigned personnel.
• Performs Phlebotomy as needed.
• Supports agency in assigned audits and office duties.
• Adheres to E-way standards in all duties.
