HealthFlex seeks an After Hours Licensed Vocational Nurse (LVN) to provide skilled, compassionate home hospice care across the Bay Area. You will support end-of-life goals, assist the RN with the plan of care, and travel as needed to cover shifts.

The role requires a current CA LVN license, hospice/home health experience, CPR certification, and strong communication skills. Travel and weekend coverage are included as part of the schedule.

Qualifikationen

• Active LVN license in California.
• Hospice experience required.
• 1+ years of Home Health experience preferred.
• Current AHA CPR certification.
• Valid driver’s license and ability to travel across the Bay Area.
• Strong communication skills, both verbal and written.

Aufgaben

• Assist the RN in implementing the plan of care.
• Travel across the Bay Area for coverage.
• Assist with patient care and end-of-life support.
• Document progress notes and care updates.
• Participate in IDG meetings.
• Provide family education on end-of-life care.
• On-call duties including holidays.
• Engage in ongoing professional development.
