Central Coast VNA & Hospice (VNA) is seeking a Palliative Licensed Vocational Nurse in Monterey, CA for a full-time role. The LVN will work under the physician and RN Case Manager to deliver individualized care and support the treatment plan, aiming to promote comfort, independence, and quality of life.

Required: California LVN license and CPR certification; at least one year of LVN experience, preferably in home health or hospice settings.

Qualifications

• Home health care experience is preferred.
• CPR certification is required.
• Licensed Vocational Nurse license in California is mandatory.

Responsabilités

• Deliver individualized LVN care under the physician and RN Case Manager guidance.
• Support treatment plans to promote comfort, independence and quality of life.
• Assess and monitor patient status and report changes promptly to the RN Case Manager or Clinical Supervisor.
• Administer prescribed medications and treatments per laws and agency standards.
• Maintain medical supplies and coordinate replenishments as needed.
