Job summary

Apple Trees Living Facility Inc is seeking a Mental Health Professional (MHP) to support adults with behavioral health challenges in a community-based residential setting. You will lead daily meetings, provide 1:1 support, and help residents build skills toward goals.

This full-time swing-shift role (12:00pm-8:00pm) emphasizes consistency, teamwork, and crisis stabilization, with benefits eligibility after 60 days and weekend coverage when possible.

Qualifications

• LMHC or LMHCA licensure or eligibility.
• Master's degree in social work, counseling, or related field.
• RN with behavioral health experience considered.

Responsibilities

• Lead daily community meetings to support structure, communication, routines, and resident engagement.
• Provide 1:1 resident support focused on skill-building and goal progress within each resident's service plan.
• Support residents with community stabilization, including emotional regulation and safety awareness in a residential setting.
• Collaborate with the interdisciplinary team to develop and update Behavior Support Plans with measurable objectives.
• Coach direct care staff on behavioral observation, de-escalation, and consistent resident-plan responses.
• Assist with crisis response and stabilization, coordinating safety planning with leadership and external resources.
• Coordinate communication with families and external providers to support continuity of goals.
• Maintain timely documentation per facility standards, including progress notes and incident follow-up.
• Assist residents with benefits navigation and agency coordination to support stability.
• Support day-to-day unit operations as part of a hands-on residential team.
