Northwestern Medicine Delnor Hospital in Geneva, IL is seeking a Social Worker to provide medical social work services to patients, families, and staff. In collaboration with the Case Manager, you will assist patients facing acute and chronic illness, support adjustment, and participate in discharge planning.

The role requires a Master’s in Social Work, LSW/LCSW or MSW/CSW, strong communication and customer service, ability to work in a fast-paced hospital setting, and on-call responsiveness

Formación

• Master’s degree in Social Work or related field; professional licensure as LCSW/CSW is preferred.
• Strong communication, customer service, and psychosocial assessment skills.
• Ability to navigate community resources and health care regulations.

Responsabilidades

• Provide medical social work services to patients, families, and staff.
• Collaborate with Case Manager to support adjustment to illness episodes.
• Assist with discharge planning and recommendations.
• Address issues of terminality and link to community resources.
