Job summary

Northwestern Medicine is seeking a Social Worker to provide comprehensive medical social work services to patients, families, and staff within the hospital system. The role collaborates with the Case Manager to help patients adjust to illness, participate in restorative health programs, and address discharge planning needs.

The position requires a Master’s Degree in Social Work or related field, LSW/LCSW or MSW/CSW credentials, strong communication, and the ability to multitask in a fast-paced

Must possess a Master’s Degree in Social Work or related field.

Strong customer service skills and effective communication.

Responsibilities

Provide medical social work services to patients, families, and staff.

Address issues of terminality and participate in restorative health programs.
