Responsibilities

• Provides medical social work services to assist patients and families with psychosocial factors affecting care.
• Demonstrates NM values and maintains a friendly, professional demeanor.
• Provides timely information and helps patients and customers navigate hospital services.
• Collaborates with physicians and health team members to address psychosocial factors.
• Counsels patients/families for short-term and long-term planning and decision making.
• Completes required communications and participates in in-services and PI activities.
