Northwestern Memorial Hospital in Chicago is seeking a Social Worker (LSW) to conduct comprehensive psychosocial assessments, engage with patients and families, and plan appropriate care. The role collaborates with the interdisciplinary team, leads discharge planning and placement coordination, provides crisis intervention when needed, and ensures HIPAA compliance.

Must hold a MSW from an accredited program and IL LSW license.

Qualifications

• Masters Degree in Social Work from CSWE-accredited program.
• Licensed Social Worker (Illinois).

Responsibilities

• Psychosocial assessment with patient and family to determine needs.
• Develop and implement an intervention plan with goals.
• Manage discharge planning and placement coordination.
• Coordinate communication among care team, patient, and payer.
• Conduct initial screening to determine medical necessity and funding.
• Provide crisis intervention and protective services as needed.
• Educate patients and families to promote wellness and system navigation.
• Ensure compliance with policies, HIPAA, and ethical guidelines.
