Job summary

Northwestern Medicine is seeking a Social Worker to provide medical social work services to patients, families, and staff, in collaboration with the Case Manager to support discharge planning and adjustment to illness. The role includes on-call responsibilities and working mostly 4PM-1AM, with some weekend on-call hours.

The position requires a Master’s in Social Work (MSW) and LSW/LCSW or MSW/CSW credentials, plus strong communication, ethics, and teamwork skills in a fast-paced hospital

Qualifications

• Must have a Masters Degree in Social Work or related field.
• Must be an LSW/LCSW or MSW/CSW.

Responsibilities

• Provide medical social work services to patients, families, and staff.
• Collaborate with the Case Manager to aid discharge planning and adjustment to illness or disability.
• Assist with coping strategies, restorative health programs, and terminality issues.
