Qualifications

• Must hold a valid LPN license in New Jersey.
• CPR certification required.
• Valid driver’s license and own transportation to multiple sites.

Responsibilities

• Administer total patient care under supervision of a Registered Nurse.
• Provide direct patient care within scope of practice.
• Document all aspects of patient care and communicate status to care team.
• Instruct patients and significant others on health state and plan of care.
• Assist in discharge planning and home health maintenance.
