Job summary

Qualifications

Responsibilities

• Administer total patient care under RN supervision.
• Assist in data collection related to patient status and needs.
• Provide direct patient care within scope of practice per plan of care.
• Instruct patients/families on health status and care plan.
• Assist discharge planning and home health maintenance.
