Qualifications

• CPR certification is required.
• A valid driver’s license is required.

Responsibilities

• Participates in data collection related to the patient status and needs and informs RN for plan of care.
• Provides direct patient care within the scope of practice to implement the plan of care.
• Informs and educates patients and significant others on health status and care plan.
• Assists patients and families in discharge preparation and home health maintenance.
• Documents all aspects of patient care.
