Job summary

Masonicare at Bishop Wicke in Shelton, CT is seeking an LPN for the evening shift to provide nursing care under the supervision of an RN, in a long-term care and rehab setting.

This role involves medication administration, contribution to care plans, coordinating with the interdisciplinary team, documenting in the EMR, supervising CNAs, and participating in ongoing education and quality improvement activities.

Qualifications

• Graduate of an accredited school of nursing for practical nursing.
• 1+ years clinical experience in a hospital or similar health care setting preferred.
• Current State of CT license to practice as a licensed practical nurse.

Responsibilities

• Provides nursing care under the supervision of an RN, for assigned patients in assigned areas practicing Evidence-Based nursing as the standard.
• Provides medication & treatment administration.
• Contributes to the care plan development by collecting and recording data.
• Communicates and collaborates with residents, families, colleagues and the interdisciplinary team.
• Documents information in EMR and supervises CNAs to ensure plan adherence.
