Job summary

Masonicare at Bishop Wicke in Shelton, CT is seeking a Per Diem Licensed Practical Nurse. The role provides nursing care under RN supervision for assigned patients, following evidence-based nursing practices.

Minimum qualifications include graduation from an accredited practical nursing program and CT licensure. Acute care experience is preferred and the position requires collaboration with an interdisciplinary team and adherence to care plans and documentation standards.

Qualifications

• Graduate of an accredited school of nursing for practical nursing.
• 1+ years clinical experience in a hospital or similar health care setting preferred.

Responsibilities

• Provides nursing care appropriate to the age, needs and abilities of the resident/patient.
• Medication & treatment administration.
• Teaching.
• Implementation of the interdisciplinary care plan.
• Collects and records data in an ongoing and systematic manner.
• Communicates and collaborates with residents/patients, families, colleagues and the interdisciplinary team.
• Documents information in EMR in a timely and concise manner.
• Supervises CNA care to assure compliance with the care plan and resident profile.
• Participates in educational programs, inservices, meetings and workshops.
• Participates in quality improvement programs as directed.
• Follows the Diligent Minimal Lift Program standards.
• Performs other duties as required.
