Job summary

Creative Solutions Services, LLC is seeking an LPN/LVN for an onsite 9-week contract in Milford, NH. Shift 3:00 PM - 11:30 PM, 8 hours/day, 5 days/week, with a 40-hour minimum guarantee. The LPN will provide direct patient care under RN supervision and document care activities per facility policy.

The role requires nursing licensure in the state, familiarity with medication administration, and the ability to work collaboratively with unlicensed staff to achieve patient goals.

Qualifications

• Graduate of an approved school for practical or vocational nursing is required.
• Must be currently licensed by the State Board of Nursing.
• Experience in rehabilitative or geriatric nursing preferred.

Responsibilities

• Collects, reports and documents objective and subjective data.
• Observes conditions and reports changes in condition to RN.
• Administers medications and performs treatments per physician orders.
• Communicates pertinent data to RN and/or physician.
• Documents accurately and thoroughly.
• Collaborates with other departments to provide timely care.
