Job summary

Fives DyAG Corp. is seeking a Licensed Practical Nurse (LPN/LVN) for an onsite assignment in Keene, NH. The eight-week contract runs from 10/05/2026, with a 3:00 PM–11:30 PM shift.

Base pay is $28.18/hour ($901.76 weekly). Schedule is 8 hours/day, 4 days/week, with a minimum 32 weeks guaranteed for the assignment. The LPN will provide direct patient care under RN supervision, participate in care planning, document data, and ensure safety and quality of care.

Qualifications

• Graduate of an approved school for practical or vocational nursing.
• Must be licensed by the State Board of Nursing.
• Current BLS/CPR certification as required by facility policy.
• Read, write, speak and understand English to ensure patient safety.
• Experience in rehabilitative or geriatric nursing preferred.
• Provide verification of tuberculosis test (TST) as required by state law.

Responsibilities

• Collects, reports and documents objective and subjective data.
• Observes changes in patient condition and notifies RN.
• Administers medications and treatments per physician orders.
• Provides direct patient care in collaboration with the RN.
• Communicates data and contributes to care planning with the team.
• Maintains patient confidentiality and PHI confidentiality.
