As a Evening LPN, you will play a vital role in providing high-quality patient care and support to our residents. In this role, you will work closely with our interdisciplinary team to deliver exceptional care, promote independence, and enhance the overall well-being of our patients.
Provide direct patient care under the supervision of registered nurses and physicians, including administering medications, performing vital signs, and assisting with daily living activities
Develop and implement individualized care plans to meet the unique needs of each patient
Monitor and report patient progress, identifying areas for improvement and implementing changes as necessary
Collaborate with the interdisciplinary team to ensure comprehensive care and coordination of services
Maintain accurate and up-to-date records of patient care and treatment
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Provide education and support to patients and their families on health-related topics and disease management
Current LPN licensure in the state of Ohio
1-2 years of experience in long-term care or a related field
Ability to work effectively in a fast-paced environment with minimal supervision

Ability to lift, transfer, and reposition patients as needed
