Pacific Clinics in Los Angeles is seeking a Housing Monitor to provide client advocacy, case management, and benefits linkage within a housing program. You will collaborate with the Housing Supervisor to support individuals facing housing-related challenges and coordination with health and social services.

The role demands strong communication, adherence to HIPAA, and the ability to work within a multidisciplinary team to ensure client safety and program success.

Qualifications

• Requires High School Diploma or GED and 2+ years of related experience or equivalent.
• Experience in social services and with homelessness populations preferred.
• Bilingual ability preferred.
• CPR/First Aid certification required (or willingness to obtain).
• Must pass DOJ/FBI background check.

Responsabilités

• Promotes a strength-based environment to help clients achieve goals.
• Monitors client compliance with program rules and policies.
• Conducts intakes, screenings, and client move-ins/move-outs.
• Provides case management support, including independent living skills and benefits linkage.
• Uses de-escalation and crisis intervention techniques when needed.
• Maintains cleanliness and sanitation of common areas, kitchen, restrooms, and client rooms.
• Conducts wellness checks, facility inspections, and perimeter checks to ensure safety.
• Completes all required documentation in compliance with regulations and timelines.
• Coordinates with outside agencies to assist clients in accessing benefits and services.
