Pioneer Valley Hospice & Palliative Care is seeking a dedicated Registered Nurse to join our Massachusetts team. The RN will triage, coordinate, and deliver hospice care, guiding plans of care from referral through discharge with an emphasis on patient dignity and comfort.

Qualifications

• Associate degree in nursing required.
• Massachusetts RN licensure required.
• Current CPR certification required.
• Hospice and Palliative Care Nurse Certification preferred.
• 1–2 years healthcare experience minimum.
• Hospice/home health experience preferred.

Responsibilities

• Triage, coordinate, and administer hospice care for patients and families.
• Manage the plan of care from referral through discharge in line with regulations and standards.
• Complete initial nursing assessment and physical examination.
• Administer medications and document responses to interventions.
• Collaborate with interdisciplinary team to educate patients and families on pain and symptom management.
• Document clinical notes, outcomes, and handoffs accurately.
• Discharge planning and communication with IDT and physicians.
