Beth Israel Lahey Health in Massachusetts is seeking an Evening Hospice RN to cover two evenings per week from 4:00pm to 9:00pm. The role delivers comprehensive patient care using the nursing process and emphasizes pain and symptom management.

You will assess patients and families, develop care plans, educate on medications, document services, supervise Hospice Aides every two weeks, and participate in bi-weekly case conferences to coordinate care with the interdisciplinary team.

Qualifications

• Active Massachusetts RN license is required.
• Associate's Degree in Nursing or BSN preferred.
• BLS certification is required.
• At least 1 year of direct care nursing experience, hospice or home care preferred.

Responsibilities

• Plan and deliver care using the nursing process (assessment, planning, intervention, evaluation).
• Interact with patients, families, and the interdisciplinary team.
• Complete initial patient and family assessment to determine hospice needs.
• Perform complete physical assessment and review medical history.
• Develop patient-specific care plans focusing on pain and symptom management.
• Provide medication education: actions, side effects, dosage, route and frequency.
• Document services using problem-oriented records.
• Instruct and supervise Hospice Aides; supervise every two weeks minimum.
• Participate in bi-weekly hospice case conferences.
