Job summary

Advocate Health is seeking a compassionate RN for its Wisconsin hospice division in Sheboygan. The role supports a 4p-12a schedule, Monday through Friday with rotating holidays, and covers patients in Sheboygan/Manitowoc areas.

External candidates may qualify for a $10,000 sign-on bonus and full-time benefits. Responsibilities include developing therapeutic nurse-patient relationships, delivering individualized care, coordinating with the care team, and ensuring quality care with safety

Qualifications

• Graduate from an accredited school of nursing.
• Minimum 1 year of experience.
• Earn 10.0 continuing education credits annually
• Has demonstrated proficiency for the requirements for NC I

Responsibilities

• Develop and maintain a therapeutic nurse-patient relationship.
• Provide individualized care to promote physical and psychosocial comfort.
• Assess patient and family readiness; involve them in care planning.
• Collaborate with physicians and other health professionals on plans of care.
• Support patient and family through illness and end-of-life care.
• Ensure quality and safety by adhering to standards and reducing risk.
• Coordinate discharge planning and referrals.
