Job summary

Advocate Health is seeking a full-time Registered Nurse to join the Milwaukee North 12 - Hospice team in Milwaukee. Schedule is Monday–Friday 0800–1630 with optional on-call and after-hours shifts; a $10,000 sign-on bonus is available for eligible external candidates.

You will coordinate patient and family care in hospice settings, educate on disease trajectories, document in EHR, supervise Home Health Aide staff, and participate in discharge planning while meeting RN licensure and BLS

Qualifications

• Active RN license in the applicable state.
• BLS for Healthcare Providers certification required on hire.
• Education in nursing from an accredited program.

Responsibilities

• Identify and coordinate patient/family care in hospice settings.
• Educate patients and families on disease trajectory and end-of-life care.
• Maintain documentation in the electronic health record and coordinate with the IDG.
