Qualifications

• Current RN license in Wisconsin.
• Minimum of 1 year of nursing experience.
• Strong nursing assessment, critical thinking and communication skills.
• Compassionate, empathetic and adaptable approach to patient care.
• Valid driver’s license, reliable transportation, and proof of auto insurance.

Responsibilities

• Provide as-needed nursing visits and/or on-call coverage.
• Conduct symptom management and comfort visits.
• Perform change-in-condition assessments and urgent visits.
• Attend death/pronouncement visits as needed.
• Travel to patients in the community and coordinate care.
