Job summary

Mountainside Treatment Center in Canaan, CT is seeking an Extended Care Coordinator for a temporary 3-6 month assignment. The role focuses on providing a welcoming, supportive, and structured living experience that meets client expectations for service during Extended Care.

You will coordinate admissions, transportation for appointments, and ensure room readiness, safety checks, and nightly routines with a focus on compassionate, best-in-class service.

Qualifications

• Compliance with federal, state and accreditation requirements.
• Effective communication with designated Managers.
• Ability to handle day-to-day non-therapeutic client needs with professionalism.
• Willingness to assist with admissions, transportation and safety checks.

Responsibilities

• Comply with federal, state and accreditation requirements.
• Take direction from designated Managers and communicate effectively with them.
• Address day-to-day non-therapeutic needs of clients with a calm, professional demeanor.
• Prepare admissions materials and ensure rooms are clean and ready for new clients.
• Complete admissions, greet clients and families, and complete necessary forms.
• Orientate new clients to facility, programs and services.
• Coordinate transportation for outside appointments, pickups and drops for new admissions and current clients.
• Assist with processing UTOX testing samples.
• Assist with morning wake-up and evening bed checks of clients.
• Perform routine safety checks of client rooms and report any infractions of guidelines.
• Support store runs and transportation to stores on designated nights.
• Coordinate prescription pickups and related tasks.
• Provide support to team members to ensure best-in-class service.
