Job summary

Mountainside Treatment Center in Canaan, CT is seeking an Extended Care Coordinator (Per Diem) to support client reintegration through therapeutic follow-up and mentorship. The role involves coordinating transportation, admissions, and daily operations to uphold Best in Class service standards.

The position requires a high school diploma, a valid driver’s license and clean driving record, with pay between $17 and $22 per hour and a per diem schedule. Benefits include paid sick time and more.

Qualifications

• High School Diploma or Equivalent is required.
• Valid Driver's License and a Clean Driving Record are required.

Responsibilities

• Comply with all federal, state and accreditation regulatory requirements.
• Take direction from designated Managers and communicate in an effective and timely manner.
• Address day-to-day non-therapeutic needs of clients while maintaining a calm, welcoming and professional demeanor.
• Welcome new admissions by ensuring materials are in place and rooms are clean.
• Complete admissions process, including greeting new clients and families and completion of forms.
• Orientate new clients to facility, programs and services.
• Coordinate transportation for outside appointments, pickups and drop offs for admissions and current clients.
• Assist with administration and processing of UTOX testing samples.
• Assist with morning wake-up and evening bed checks.
• Perform routine inspections of client rooms and report infractions.
• Assist with store runs and transportation for clients.
• Facilitate prescription ordering and pickup for clients.
• Support the team to ensure unsurpassed service and assist in the recovery process.
• Must drive safely and without incident.
