Job summary

Mountainside Treatment Center in Canaan, CT is seeking an Extended Care Coordinator (Per Diem) to guide clients through daily living, coordinate transportation, and ensure a smooth admission and ongoing support.

You will work under supervision to comply with regulatory requirements, assist with admissions, manage wake-up and bed checks, and help maintain a safe, clean environment while delivering Best in Class service.

Qualifications

• High School Diploma or Equivalent is required.
• Valid Driver's License and Clean Driving Record are required.

Responsibilities

• Comply with all federal, state and accreditation regulatory requirements.
• Coordinate transportation for outside appointments, pickups and drops for new admissions, current clients and discharging clients as assigned.
• Assist with morning wake-up and evening bed check of clients as necessary.
• Perform routine inspections of client rooms, reporting any infractions of ECare resident guidelines.
• Assist with store run purchases on behalf of clients providing transportation to stores on designated nights.
• Facilitate inspection, ordering and pick up of prescriptions for clients.
• Provide passengers with assistance entering and exiting vehicles and help them with any luggage.
• Performs safety checks of house and perimeter throughout the night.
• Reports defects in the physical property, submits requests for maintenance and repairs.
