Job summary

Sevita in Indiana is seeking a Direct Support Professional to provide person-centered services in residential settings, supporting 4-6 individuals. You will assist with daily living activities, health maintenance, and community integration.

Responsibilities include following individual plans, documenting progress, supporting health care needs, and ensuring safety. A high school diploma and 18+ years old are required; prior human services experience is preferred.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Direct Support: assist with socialization, personal care, transportation, and daily living activities.
• Records: document progress and maintain confidentiality.
• Management of Assets: make budget-conscious purchases per individual plans.
• Relationships: maintain professional relationships with individuals, families, and guardians.
• Reporting: report incidents per Sevita policy and external requirements.
• Rights: protect confidentiality and individual rights.
• Appointments: accompany to medical visits when needed; relay information.
• Medications: support administration or self-administration; document delivery.
• General Health Care: monitor health and communicate with nurse or supervisor.
• Meal Preparation: assist with meals per planned menus and dietary needs.
• Training: attend orientation and ongoing training; participate in staff meetings.
• Teamwork: collaborate with others; communicate openly and respectfully.
• Workplace Safety: follow safety policies and report hazards.
• Maintenance: assist with safe driving and community transport if required.
