Job summary

Sevita seeks Direct Support Professionals to assist individuals with intellectual and developmental disabilities in residential settings. You will implement individualized plans, help with daily activities, and support health and safety needs.

Responsibilities include documentation, safety, and coordination with nurses, supervisors, and family members. Must be at least 18, have a driver’s license, and basic computer skills. Competitive schedule and comprehensive training offered.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Direct Support: provide assistance per service plans, including socialization, health maintenance, medication administration, and skill development.
• Records: accurately document progress and activity; maintain confidentiality.
• Management of Individuals’ Assets: make cost-effective purchases within the individual’s budget and preferences.
• Relationships: maintain professional relationships with individuals, families, guardians and case managers; adhere to Sevita Customer Service Standards.
• Reporting: report abuse or neglect and medical/behavioral incidents per policy.
• Rights: protect confidentiality and support individuals’ rights.
