Job summary

Hope Haven, Inc. seeks a Direct Support Professional to assist clients in achieving personal goals and ensuring their physical, mental, emotional, and social needs are met. Shifts are 5:30pm–8:00pm, 3 days per week, offering an entry path into Human Services with a meaningful, day-to-day impact.

Responsibilities include providing care per service plans, documentation, and promoting community participation while maintaining safety and teamwork with staff and clients.

Qualifications

• Good written and verbal communication skills.
• Basic computer skills required.
• Valid Class C driver’s license required (Class D if vehicle capacity is 8+).
• Must pass health assessment at time of hire and maintain as required.
• Background checks as required by service standards.
• Reliable transportation.
• Proof of state minimum liability insurance coverage.

Responsibilities

• Support individuals to participate in daily activities and community interactions.
• Provide training and support per service plan, including personal care as needed.
• Teach self-management and self-control per training.
• Complete client logs and charts within 24 hours with supervisor approval.
• Arrange appointments and transportation with proper documentation.
• Promote safety and follow all safety procedures.
• Be a TEAM PLAYER with positive communication.
