Job summary

Hope Haven is seeking a Direct Support Professional to assist individuals with developmental needs in daily living and community activities.

You will provide privacy-respecting support including personal care, scheduling, and transportation assistance while documenting activities and progress. The role emphasizes safety, reliable communication, and teamwork in a supportive environment.

Qualifications

• High school diploma or GED/HSED required.
• Good written and verbal communication skills and basic computer skills.
• Ability to exercise sound judgment in decisions.
• Must pass required medication administration course.
• Valid driver’s license required and able to meet health assessment standards.
• Clearance from background checks as required by service standards.

Responsibilities

• Support individuals to participate in daily activities and community involvement.
• Provide care including hygiene, escort, mealtime assistance as per service plan.
• Teach self-management and self-control per training.
• Complete client logs and charts within shift or within 24 hours.
• Assist with scheduling and transportation and document accordingly.
• Promote personal safety and follow safety procedures.
