Job summary

Fox Hollow Post Acute is seeking a Certified Nursing Assistant for full-time evening shift (2pm-10pm) to deliver routine daily care to residents under the supervision of nursing staff. The role emphasizes accurate documentation, adherence to care plans, and maintaining a safe, supportive environment for residents in our post-acute facility.

Ideal candidates hold a state CNA license, have a minimum of a 10th grade education, and demonstrate the ability to read and follow care instructions.

Qualifications

• Must possess a Certified Nursing Assistant license in accordance with state laws.
• Minimum of a 10th grade education.
• CPR certification preferred.
• Ability to read, analyze, and interpret general business materials and communicate effectively.

Responsibilities

• Provide routine daily nursing care and services per resident care plans.
• Identify residents before treatments and meals using wristband/photo ID.
• Record entries accurately on flow sheets and charts.
• Report changes in resident condition to Nurse Supervisor promptly.
• Maintain resident room readiness and assist with transfers and mobility as needed.
