PharMerica in Greensburg, PA is seeking a full-time Pharmacist to join a non-retail, long-term care focused team. You will evaluate orders, provide clinical recommendations, and maintain patient records, ensuring safe, compliant medication management in a collaborative setting.

Qualifikationen

• Bachelor’s Degree in Pharmacy; PharmD preferred.
• One year experience in long term care desired.
• Current and valid pharmacist license in state of practice.
• Knowledge of drug therapy guidelines, safety practices and CMS/DEA/FDA standards.
• Proficiency in EMR and pharmacy information systems.
• Strong clinical judgement and effective communication.

Aufgaben

• Evaluate medication orders for appropriateness, dosage, and interactions.
• Provide clinical recommendations on therapy changes to physicians.
• Maintain patient profiles with history, labs, and medications.
• Order and review labs for high‑risk medications and communicate results.
• Collaborate with physicians, nurses and staff for clinical support.
• Document all interventions in the patient’s medical record.
• Enforce safe handling, dispensing and oversight of medications.
• Ensure compliance with state and federal regulatory requirements.
• Report medication errors to Pharmacy Manager as required.
• Educate and support clinical staff on safe medication handling.
• Perform other pharmacy tasks as assigned.
