• Provide clinical recommendations to physicians and healthcare staff.
• Maintain patient profiles with medications, labs, and history.
• Ensure monitoring tests for high‑risk meds are ordered and reviewed.
• Collaborate with physicians and nurses for therapy optimization.
• Document all clinical interventions in the medical record.
