Qualifications

• Proficiency with electronic medical records and pharmacy information systems.
• Strong clinical judgement and communication skills.
• Pharmacist license in state of practice.

Responsibilities

• Evaluate medication orders for appropriateness, dosage, and interactions.
• Provide clinical recommendations to physicians and staff.
• Maintain patient/resident profiles with medical history and medications.
• Ensure lab tests for monitoring high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses and healthcare professionals.
• Document communications and interventions in medical records.
• Ensure compliance with regulatory requirements and safe medication handling.
