Job summary

Community Mental Health Affiliates, Inc. (CMHA) seeks a Full-time Case Manager for the Transitional Living Center in New Britain, CT.

This 2nd shift role (Sunday-Thursday, 3:30 PM–11:30 PM) focuses on connecting homeless clients with mental health and MAT services, coordinating care with external providers. You will advocate for clients, help with housing, finances, and daily living skills, and document progress in CMHA's system.

Responsibilities

• Provides advocacy, support and guidance with contacting external organizations (Social Security, DSS, DCF, DMHAS, etc).
• Educates clients to utilize emergency and community resources to help meet basic needs.
• Works with clients to teach life skills necessary to live independently in the community. This includes assisting with finances and entitlements, supporting the development of activities of daily living, nutrition, housing maintenance and repair, and prompting to encourage task completion.
• Collaborates and integrates with other CMHA teams to ensure all services provided to client are communicated with clients whole treatment team
• Assures timely documentation is maintained in accordance with CMHA policies and procedures, including but not limited to progress notes, treatment plans, authorizations, program orientation, and consent to treat, activity logs, and monthly targeted case management notes. Maintains appropriate caseload as determined by CMHA and funder
• Performs other related duties as assigned.
