Job summary

Responsibilities include using basic clinical interventions to support sobriety and mental health, maintaining up-to-date resources, submitting progress notes within 24 to 48 hours, and collaborating with referral sources to coordinate care.

Qualifications

• High School Diploma or equivalent.
• 3+ years of experience in social work.
• Chemical Dependency certification or Social Work licensure preferred.
• Must be certified within 90 days of starting on First Aid, CPR, and De-escalation Training.
• Have a valid driver's license and be willing to use a personal car for job duties.
• Computer skills including Excel for reporting.

Responsibilities

• Manage cases for assigned clients to ensure services per the client's ITP and overall wellbeing.
• Utilize basic clinical interventions to support sobriety and mental health.
• Acquire and maintain knowledge of community resources and referrals.
• Submit progress notes within 24 to 48 hours.
• Achieve or exceed individual productivity goals.
• Review case files to ensure audit-ready status.
• Collaborate with referral sources, agencies, and legal entities to coordinate client care.
• Acquire and maintain crisis de-escalation techniques for emergencies.
• Perform duties aligned with the agency's Mission and Standards of Ethical Conduct.
• All other duties as assigned.
