Responsibilities

• Guest Care & Advocacy: coordinate services and resources; advocate for individuals; assist daily living activities and linkages for education, support and independence.
• Compliance & Risk Management: document information for care coordination; stay updated on regulations and RI policies; follow conduct and conflict policies.
• Cross Functional Collaboration: coordinate multidisciplinary meetings; partner with providers to help participants achieve wellness.
• Additional Duties: perform other related duties as assigned.
