Job summary

Htprof is a mission-driven senior living and healthcare organization in Connecticut seeking an Admissions Nurse (RN) to oversee clinical intake for incoming residents during the evening shift (3:00 PM – 11:00 PM).

The role coordinates with discharge planners, physicians, families, and leadership to ensure safe transitions, medication reconciliation, and rapid chart setup, enabling staff to focus on resident care from day one.

Qualifications

• Active, unrestricted RN license in CT.
• 1–2 years of nursing experience in SNF/LTC/sub-acute rehab preferred.
• Prior admissions or discharge coordination experience is highly desirable.
• Hands-on EMR experience with PointClickCare for chart initiation.

Responsibilities

• Lead evening clinical intake and onboarding for admissions.
• Conduct initial nursing assessments and documentation on arrival.
• Reconcile hospital discharge summaries and orders for continuity.
• Coordinate with discharge planners, care teams, and families.
• Initiate preliminary care plans and inform nursing supervisors.
• Orient residents and families to policies and routines.
