Job summary

Harboroaks is seeking an Admission Counselor to support evening admissions and clinical intake assessments. You will facilitate utilization review processes to ensure appropriate care levels and patient benefits alignment.

The role emphasizes developing therapeutic relationships, coordinating admissions, and communicating with payors and families to ensure smooth transitions. A healthcare background and relevant licenses are valued.

Qualifications

• Bachelor’s degree in a social services field required.
• Master's degree in social services field (psychology, counseling, social work, sociology, health administration, nursing) or RN preferred.

Responsibilities

• Respond to inquiries about the facility within facility policy timeframes.
• Develop therapeutic relationships with patients and families.
• Perform benefit eligibility to ensure patient benefits are active at the time of admission; communicate eligibility to business office and patient/family.
• Collaborate with facility medical and psychiatric personnel for appropriate referrals.
• Coordinate admission and transfer from referral sources and between levels of care within the facility.
• Maintain knowledge of milieu management.
• Communicate projected admissions to internal representatives promptly.
• Ensure all clinical information from referral sources or patients is received prior to admission.
• Schedule and complete pre-admission assessments; consult with admitting physician and communicate disposition recommendations to patient/family.
• Prepare and maintain medical record documentation for payor authorization.
• Complete initial pre-authorization for treatment and admission within payor guidelines.
• Admit patient to registration and accounting system; complete forms with patient.
• Demonstrate empathy and professionalism; address complaints and prioritize patient safety.
• Coordinate care for patients not admitted and arrange follow-up care and referrals.
• Demonstrate urgency regarding patient safety and provide excellent customer service.
• Conduct safety checks and supervise per 15-minute intervals or as required.

Education

Bachelor’s degree in social services

Master's degree in social services or RN preferred
