Job summary

University Health System- San Antonio is seeking a qualified individual to assist Social Workers and Case Managers in coordinating care across the continuum, addressing psychosocial and socioeconomic needs of patients and families.

Collaborating with case managers, physicians, staff, community resources and payers, the role ensures timely care, regulatory compliance and efficient use of hospital resources while completing required competencies during orientation.

Qualifications

• High school diploma required.
• Associates degree preferred.
• Six months experience in a health care environment required.
• Preferred experience in discharge planning in HMO, hospital, home health or discharge planning settings.

Responsibilities

• Coordinate care with social workers and case managers to address psychosocial and socioeconomic needs across the care continuum.
• Ensure timely care, regulatory compliance, and efficient use of hospital resources.
• Complete established competencies within designated introductory period.
