Responsibilities

• Coordinates and delivers high-quality, patient-centered care in collaboration with medical providers and care team.
• Assesses patient pain regularly and promotes effective pain management with timely reassessments.
• Documents administration of care in the patient medical record in a timely manner.
• Responds promptly to patient requests and anticipates needs to ensure exceptional care.
• Educates patients/families/caregivers about condition, plan, medications, and follow-up measures.
