Job summary

Qualifications

Responsibilities

• Assessment: performs accurate multi-system assessments and discharge planning needs.
• Planning: develops an individualized nursing plan of care and coordinates with departments and community agencies.
• Implementation: administers medications, follows physician orders, documents in EMR.
• Evaluation: assesses outcome of care and participates in unit quality assurance activities.
