Job summary

UMass Memorial Health seeks a Point of Entry/Emergency Department Case Manager (POE/ED CM) for the Milford Regional ED. You will coordinate care for ED patients, assess needs on intake and support discharge planning as part of a collaborative healthcare team.

Required BSN and MA RN license with a minimum of 3 years in medical/surgical/ICU or ED nursing. CCM certification is a plus; position is non-exempt with weekend rotations and 12-hour day shifts.

Qualifications

• BSN required with active MA RN license.
• CCM certification preferred.
• Minimum 3 years in medical, surgical, ICU or ED nursing.
• Strong communication, analytical and teamwork abilities.

Responsibilities

• Coordinate care for ED patients and facilitate discharge planning.
• Evaluate patient needs, communicate with care teams and families.
• Assist in admission decisions using InterQual criteria.
• Collaborate with ED, MSW and admitting teams for smooth transitions.
• Document reviews and ensure timely escalation when needed.
