TriStar Centennial Medical Center in Tennessee seeks an ER Discharge Coordinator (PRN) to manage bedside follow-up scheduling as part of centralized ER and Urgent Care programs. The role focuses on a patient-centered, quality-driven experience and timely coordination of care.

Responsibilities include professional communication with patients and staff, scheduling follow-up care, maintaining service standards, and contributing ideas to improve department policies. Prior experience is preferred.

*This position requires providing coverage at area medical centers including: Centennial, Hendersonville, Summit and Southern Hills

Job Summary and Qualifications

As an ER Discharge Coordinator, you will be responsible for Point of Care (PoC) Patient Bedside scheduling as part of the company’s centralized ER and Urgent Care Follow-up programs. The truest intent of the ER discharge coordinator is a bedside-initiated patient experience, with the objectives of increased contact rate, scheduling immediacy, and improved overall patient experience. You will create and maintain a positive customer experience in a patient-centered, quality-driven environment.

In this role you will:
