Job summary

Boston Medical Center (BMC) seeks an experienced RN Care Manager for the Emergency Department. The role focuses on assessing, planning, implementing, monitoring, and evaluating care options to meet patient health needs using available resources to promote quality outcomes.

Responsibilities include identifying appropriate providers, coordinating across the continuum, and supporting the department’s care management process to handle rising volumes.

Qualifications

• Nursing degree required; BSN preferred; ability to obtain BSN within 4 years.
• License to practice as RN in MA by start date.
• CCM or related certification preferred within 24 months from hire date.
• 3-5 years clinical experience; 1 year case management required; ED experience preferred.
• Knowledge of managed care concepts and ICD-9/CPT terminology.
• Excellent interpersonal, verbal and written communication and negotiations skills.

Responsibilities

• Assess, plan, implement, monitor, and evaluate care options across the continuum.
• Coordinate with providers and facilities to ensure timely, cost-effective care.
• Support the care management process and department to handle increased volume.
