Job summary

Qualifications

• Nursing degree required; Diploma, ASN or BSN preferred, with BSN achievable within 4 years.
• Licensed RN in Massachusetts by start date; CCM or related certification preferred within 24 months.

• Identify appropriate providers and facilities to optimize value for patient and payer.
• Support increased volume and overall care management processes within the department.
